Simple Present

Simple Future
Present Continouos
Past Continuous
Present Perfect
Past Perfect
Future Perfect

Sentence: I read a book.

Simple Present
Active: I read a book.
Passive: A book is ead by me.

Simple Past:
Active: I read a book.
Passive: A book was read by me.

Simple Future:
Active: I shall read a book.
Passive: A book will be read by me.

Present Continuous:
Active: I am reading a book.
Passive: A book is being read by me.

Past Continuous: 
Active: I was reading a book.
Passive: A book was being read by me.

Present Perfect
Active: I have read a book.
Passive: A book has been read by me.

Past Perfect:
Active: I had read a book.
Passive: A book had been read by me.

Future Perfect:
Active: I shall have read a book.
Passive: A book will have been read by me. 

Passive form is not applicable for 
Future Continuous
All Present Perfect Tenses.
